Web-based ICF-based Documentation Form

The World Health Organization's International Classification of Functioning, Disability and Health (ICF) is the common framework to understand and describe functioning and disability. To make the ICF applicable in practice and to allow a user-friendly description of functioning and disability, ICF Core Sets have been developed. ICF Core Sets facilitate the description of functioning in clinical practice by providing lists of categories that are relevant for specific health conditions and health care contexts.

There is a web-based documentation form (www.icf-core-sets.org) available to facilitate the description of functioning based on ICF Core Sets in clinical practice. This documentation form is available in English, German, French, Spanish, Chinese and Finnish.
